In order to serve minority-owned businesses and entrepreneurs, you must provide us with a completed Request for Assistance form. The form is in Adobe PDF format and requires the free Adobe Acrobat Reader to be viewed and printed. Please download the form, print it out, and fax it to (206) 267-3132. You can also bring a completed form when you visit our office or meet with one of the visiting Business Consultants in your area. (NOTE: Appointments must be made in advance before meeting with a Business Consultant.)
